- Student鈥檚 Immunization Records: New 小黄鸭导航 ISD families need to provide their child鈥檚 current immunization records. Returning families only provide immunization records if updated.
- Parent or Guardian Identification: New 小黄鸭导航 ISD families need to provide a parent or guardian ID. Returning 小黄鸭导航 ISD families only need to provide a parent or guardian ID if it is new or updated.
- Proof of Guardianship: New and returning 小黄鸭导航 ISD families need to provide proof of guardianship only if applicable or updated.
- Proof of Residency: New and returning 小黄鸭导航 ISD families need to provide proof of residency every year. This can be an electric bill, gas bill, lease, or mortgage statement that is current and dated within 45 days.
In addition, new families will need:
- Child鈥檚 Birth Certificate
- Child鈥檚 Social Security Card (optional)
- Child鈥檚 withdrawal form or last report card (if applicable)
